Abstract

Background

The ischaemic stroke (IS) is one of the most frequent cause of death in Germany. Besides of non-drug many drug-based interventions are used in primary or secondary prevention of IS, among them the thrombocyte aggregation inhibitors (TAI).

Objectives

The evaluation addresses the questions on medical efficacy and cost-effectiveness of the TAI administration in the prevention of IS as compared to the management of risk factors alone as well as to the use of anticoagulant drugs.

Methods

The literature search for articles published after 1997 was conducted in December 2003 in the most important medical and economic databases.
The medical analysis was performed on the basis of the most up-to date meta-analyses of randomised controlled trials (RCT) as well as of new published RCT. The data from the studies for stroke, bleeding complications as well as for the combined endpoint "severe vascular events" (SVE: death or stroke or myocardial infarction) were summarised in meta-analyses.
In order to include grey literature contact has been taken up with the pharmaceutical manufacturers of TAI. Results are presented in a descriptive way.

Results

The medical analysis included data from 184 RCT (vs. placebo) and from 22 RCT (vs. anticoagulant drugs). The absolute reduction of IS (4.8% vs. 6.6%; p<0,00001) and SVE (10.0% vs. 12.4%; p<0,00001) were definitely higher than the absolute increase of bleeding complications (1.6% vs. 0.9%; p<0,00001), but relatively similar to this absolute increase in a subpopulation with a low risk for SVE. With regard to the stroke prevention, evidence of efficacy could be yielded for acetylsalicil acid (ASA), dipyridamole, cilostazol, ridogrel and the combination ASA with dipyridamole. ASA is less effective than anticoagulants in the prevention of ischaemic stroke in atrial fibrillation, however, it causes fewer bleeding complications.
Low dosed ASA can be considered cost-effective in secondary prevention of ischemic stroke, which is not the case for clopridogrel. Dipyridamole/ASA being more effective compared with ASA alone is connected with higher acquisition costs.

Discussion

The exclusive consideration of stroke prevention is limited, as well as physicians by allocation to TAI expect to avoid all thrombotic events.
Since no pharmacoeconomic studies exist for the German context, the economic assessment had to be based on international evidence.

Conclusions

From the medical point of view, TAI may be recommended for primary and secondary prevention of IS in patients with a high risk of severe vascular events and with low risk for bleeding complications. ASA may be also administered in patients with atrial fibrillation in case of contraindication to anticoagulation drugs.
From the health economic point of view a systematic primary prophylaxis of IS with ASA cannot be recommended, whereas in secondary prophylaxis savings can be made. Neither the use of Dipyridamole/ASS nor Clopidogrel is advisable for a systematic secondary prophylaxis of IS.

Zusammenfassung

Wissenschaftlicher Hintergrund

Der ischämische Schlaganfall (IS) ist eine der häufigsten Todesursachen in Deutschland. Außer nicht-medikamentösen werden auch verschiedene medikamentöse Maßnahmen zur Primär- bzw. Sekundärprävention des IS eingesetzt, darunter die Anwendung von Thrombozytenaggregationshemmern (TAH).

Forschungsfragen

Es sollen die medizinische Effektivität und der Kosteneffektivität des Einsatzes von TAH in der Prävention des IS im Vergleich zum alleinigen Management von Risikofaktoren sowie im Vergleich zu Antikoagulantien bewertet werden.

Methodik

Die Literaturrecherche fand im Dezember 2003 in den wichtigsten medizinischen und gesundheitsökonomischen Datenbanken mit Referenzzeitraum ab 1997 statt.
In die medizinische Bewertung wurden Übersichtspublikationen zum aktuellsten Wissensstand sowie neu publizierte RCT eingeschlossen. Daten für Schlaganfälle, Blutungskomplikationen sowie für den kombinierten Endpunkt "schwere vaskuläre Ereignisse" (SVE) wurden in Metaanalysen ausgewertet.
Im Rahmen der gesundheitsökonomischen Analyse wurde außerdem Kontakt mit den TAH-Herstellern zur Identifikation unpublizierter Studien aufgenommen. Die Ergebnisdarstellung erfolgt deskriptiv.

Ergebnisse

Es wurden insgesamt Daten aus 184 RCT (vs. Placebo) und aus 22 RCT (vs. Antikoagulantien) in die medizinische Bewertung einbezogen. Die absolute Reduktion IS (4,8% vs. 6,6%; p<0,00001) und SVE (10,0% vs. 12,4%; p<0,00001) war deutlich größer als die absolute Zunahme an Blutungskomplikationen (1,6% vs. 0,9%; p<0,00001), nur in einer Subpopulation mit niedrigem SVE-Risiko dagegen annährend gleich. Hinsichtlich der Schlaganfällprävention konnte ein Wirksamkeitsnachweis für Acetylsalicylsäure (ASS), Dipyridamol, Cilostazol, Ridogrel und der Kombination von ASS mit Dipyridamol erbracht werden. ASS ist weniger wirksam als Antikoagulantien bei Vorhofflimmern, allerdings mit weniger Blutungskomplikationen.
Aus gesundheitsökonomischer Sicht ist niedrig dosiertes ASS im Rahmen der IS-Sekundärprophylaxe nicht nur kosteneffektiv, sondern es lassen sich auch Kosten einsparen. Beides trifft für Clopidogrel nicht zu. Die Kombination ASS/Dipyridamol ist bei besserer klinischer Wirksamkeit mit höheren Kosten verbunden, so dass über Art und Umfang der Anwendung die gesellschaftliche Zahlungsbereitschaft entscheidet.

Diskussion

Die exklusive Betrachtung der Schlaganfallprävention ist nur eingeschränkt hilfreich, da Ärzte bei der TAH-Verordnung von einer Vermeidung aller thrombotischen Ereignisse ausgehen.
Auch die gesundheitsökonomische Bewertung wurde durch die vergleichsweise enge Themenstellung methodisch erschwert. Zudem ist zu berücksichtigen, dass keine gesundheitsökonomischen Studien für das deutsche Gesundheitssystem vorliegen, sodass die Bewertung auf ausländischen Quellen beruhen musste.

Schlussfolgerung

Aus medizinischer Sicht können TAH zur Primär- und Sekundärprävention des IS bei Patienten mit hohem Risiko für SVE und ohne erhöhtes Risiko für Blutungskomplikationen empfohlen werden, für Patienten mit Vorhofflimmern bei Kontraindikationen für Antikoagulation.
Eine systematische medikamentöse Primärprophylaxe des IS erscheint aus gesundheitsökonomischer Sicht nicht angezeigt. Durch die Verwendung von ASS im Rahmen der IS-Sekundärprävention können Kosten eingespart werden. Für die übrigen TAH hingegen ist von einer Verwendung bei der systematischen IS-Sekundärprophylaxe Abstand zu nehmen.
Insbesondere bei Risikopatienten ist erheblicher gesundheitsökonomischer Forschungsbedarf zu konstatieren.


Executive Summary

1. Background

The ischaemic stroke (IS) is an acute focal neurological deficit due to insufficient blood circulation in the brain, which is caused itself by at least temporary closure of arterial vessels. IS is one of the most frequent cause of death in Germany. Approximately 255 strokes per 100,000 occur in Germany every year.

Many drug-based therapies in addition to non-drug interventions are used to prevent the first or recurrent IS (primary or secondary prevention of IS), - among them the use of thrombocyte aggregation inhibitors (or thrombocyte function inhibitors, TAI).

The main action mechanism of TAI is to inhibit the contact of thrombocytes with vessel endothelium, thrombocytes as well as with other blood cells. This mechanism is associated with main side effect of these substances - bleeding from the internal or external cerebral vessels. For the classic prevention (through a long-term therapy) oral TAI are of predominant importance. The daily therapy costs for the most important oral TAI (acetylsalicylic acid - ASA, ticlopidine, clopidogrel and combination of ASA with dipyridamole) range from 0.03 € (ASA) to 2.30 € (ticlopidine).

2. Objectives

The medical evaluation addresses questions on the medical efficacy of the TAI administration in the prevention of IS as compared to the management of risk factors alone as well as to the administration of anticoagulant drugs.

From an economical point of view, the cost-effectiveness of the use of TAI in the primary and secondary prevention of IS as well as their impact on health-related quality of life are of outstanding importance.

3. Medical evaluation

3.1 Methods

The literature search for articles published after 1997 was conducted in December 2003 in the most important medical databases. The analysis was performed on the basis of the most up-to date meta-analyses of ran-domised controlled trials (RCT) as well as of new RCT. The data for stroke, bleeding complications as well as for the combined endpoint "severe vascular events" (SVE: death or stroke or myocardial infarction) were extracted from the studies.

The results of the studies were checked on methodical quality, heterogeneity and then summarised in the meta-analyses. Estimates of avoided and additional events per 1000 assigned patients were calculated at a 95% confidence interval.

3.2 Results

The medical analysis included data from 184 RCT (vs. placebo) and from 22 RCT (vs. anticoagulant drugs).

The comparison of TAI with the management of risk factors alone and the comparison of different TAI regimes

In primary as well as in secondary prevention of IS the administration of TAI reduces the rate of IS by 20% to 35% (relative risk reduction) compared to the management of risk factors alone (absolute risks 4.8% vs. 6.6%; p<0.00001). This risk reduction is associated with a relative reduction in the rate of SVE (death or stroke or myocardial infarction) by 16% to 26% (absolute risks 10.0% vs. 12.4%; p<0.00001) and with a relative increase in the rate of bleeding complications by 45% to 93% (absolute risks 1.6% vs. 0.9%; p<0.00001). The calculated effect estimates may be generally applied to all patients' categories and subcategories investigated. The absolute reductions in IS and SVE are definitely higher than the absolute increase in bleeding complications in all patients' categories and subcategories, however they are relatively similar to this absolute increase in a subpopulation with a low risk of IS and SVE.
The present analysis revealed the efficacy in the reduction of SVE-rates for the following TAI: acetylsalicylic acid (ASA, over 75 mg/day), dipyridamole, ticlopidine, picotamid, trapidil, cilostazol as well as for the combination of ASA with dipyridamole. The reduction in the rate of stroke was yielded only for ASA (over 150 mg/day), dipyridamole, cilostazol as well as for the combination of ASA with dipyridamole.

No differences could be identified between any of the various applied ASA doses.

In comparison to ASA an advantage was yielded for clopidogrel (as well as for thienopyridines in general) with regard to the reduction in the rate of SVE in patients with a previous thrombotic event (IS, MI or peripheral arterial obstructive disease), for ridogrel regarding the rate of stroke in patients after MI as well as for ticlopidin, triflusal and sulfinpyrazon regarding the rate of major bleedings (in the case of triflusal only for patients after IS or TIA).

The combination of dipyridamole retard with ASA reduced the rate of stroke compared to ASA alone in patients after a previous stroke or TIA. The combination of clopidogrel with ASA showed an advantage compared to ASA alone in the reduction of SVE in patients after acute coronary syndrome without ST-segment elevation in EKG or in patients after percutaneous coronary intervention, however, it was associated with additional bleeding complications. By the combination of oral GB IIb / IIa-antagonists orbofiban or lotrafiban with ASA an increase in the rate of major bleedings without benefit in efficacy was yielded.
TAI in comparison with the use of anticoagulants

The anticoagulants showed better efficacy than TAI in patients with atrial fibrillation with regard to the rate of IS and in patients after acute cardiovascular events with regard to the rate of all SVE (at least for warfarin in comparison to ASA). Especially pronounced is the preventive effect in patients with atrial fibrillation. In contrast, the administration of ASA after acute or previous stroke or TIA compared to high-dosed anticoagulants is safer with regard to the rate of bleedings, haemorrhagic strokes and all recurrent strokes.

In patients with atrial fibrillation the pure warfarin anticoagulation (INR=2.3-2.4) is more effective as compared to the combined low-dosed combination therapy of warfarin plus ASA (INR=1.1-1.3) with regard to the rate of IS without a significant increase in major bleedings. In patients after acute myocardial infarction the combined 75 to 81 mg/day ASS plus warfarin regimes (INR=2.2-2.4) obtained similar results to those of pure warfarin anticoagulation (INR=2.8-3.2), but showed a significant increase in the rate of major bleedings in comparison to the use of ASS in a high dose (160 mg/day) alone.
3.3 Discussion

Despite potential study biases the definite impact of individual factors could not be settled. Therefore, the study results were considered as valid for the populations and technology modifications investigated (intern validity). The transferability of the results from the studies to other populations and technology modifications (extern validity) is limited.

The most up-to date publications evaluated only a part of the studies included in the present analysis. They show, however, quite similar results to those of the present report.

4. Economical evaluation

4.1 Methods

An extensive literature search was conducted in medical and health-economic data bases. Moreover, relevant health-economic journals were hand searched for studies related to the objectives of the HTA report. The articles should have been published between 1997 and 2003. Additionally, websites of respective neurological societies were reviewed for important information and pharmaceutical manufactures of TAI were contacted in order to obtain unpublished studies.

To be included in the analysis, the study should deal with costs, cost-effectiveness or cost-benefit aspects of the primary or secondary prevention of IS. Publications on acute treatment of IS and on exclusively prevention of myocardial infarction were excluded. Only publications in German, English, French and Spanish were considered. The evaluation of the identified studies was based on a 25 item criteria catalogue for methodical quality of health-economic studies. Results are presented in a descriptive way.

4.2 Results

Only three methodologically appropriate studies were identified. For four other studies this was not the case, especially due to a high degree of intransparency in pharmacoeconomical modeling. The identified studies address the use of ASA, clopidogrel as well as the combination of dipyridamole with ASA in the primary and secondary prevention of IS.

Based on identified studies, the administration of ASA in primary prevention of IS (no previous IS) appears to be cost-effective only in high-risk patients after previous myocardial infarction. The cost-savings in this patient group by the use of ASA are expected from a societal perspective. In the secondary prevention of IS the administration of ASA is associated with cost-savings in all patients without ASA intolerance. This statement is somewhat limited because of only one identified study for the endpoint IS. From the economic point of view new studies on this issue are desirable due to the political importance of IS prevention.

The use of clopidogrel is not cost-effective owing to the lack of data on beneficial medical efficacy as compared to ASA with regard to IS. Due to substantially higher costs of clopidogrel, ASA possesses a higher cost-effectiveness. Therefore, clopidogrel should be administered in the case of ASA inefficacy or intolerance.

The administration of the combination of ASA with dipyridamole in the secondary prevention of IS may be a cost-effective alternative as compared to ASA alone. The following cost-effectiveness ratios were determined:

- £ 3200 (€ 4800)/Life-year with functional restrictions,
- £ 1800 (€ 2700)/Life-year without functional restrictions,
- £ 1900/(€ 2900) per avoided stroke,
- £ 600/(€ 900) stroke-free life-year.
4.3 Discussion

While interpreting the results of economic analysis it should be considered that the applied models in the studies were not constructed for the German context. However, these results seem to be generally transferable as the costs and resource used are relatively similar in Germany. Pharmacoeconomical studies for the German context are desirable not only because of IS epidemiological importance.

5. Summarising discussion of all results

The exclusive consideration of stroke prevention is limited, as physicians expect to avoid all thrombotic events by administration of TAI. The importance of non-drug interventions has to be emphasized in the primary prevention of IS.

6. Conclusions

From a medical point of view TAI may be recommended for primary and secondary prevention of IS in patients with a high risk of severe vascular events and with a low risk of bleeding complications. At the same time, a reduction of all SVE and an increase in bleedings may be expected. The absolute reduction of thrombotic events is in general definitely higher than an absolute increase in bleeding complications. In patients with a low risk of SVE (patients without previous stroke or myocardial infarction) the difference between avoided events in relation to additional bleedings is lower, making the use of TAI in these patients less reasonable or not reasonable at all.

Generally, ASA in a dose over 75 mg/day should be administered. Other TAI such as dipyridamole, ticlopidin or clopidogrel and (due to limited data only for definite patient groups) picotamid, trapidil, cilostazol, triflusal, ridogrel or sulfinpyrazon as substitutes as well as dipyridamole or clopidogrel as additional drugs to ASA may be given. The use of ticlopidin, sulfinpyrazon and triflusal is beneficial as compared to ASA with regard to bleeding complications. Clopidogrel has an advantage in comparison to ASA in patients with previous thrombotic event with regard to the risk of all SVE without influencing the risk of stroke. The combination of ASA with oral GPII/IIIa-antagonists should only be administered in special indications due to increased risk of bleeding complications.

In patients with atrial fibrillation, ASA is definitely less efficient than anticoagulation and should only be administered in the case of increased risk of bleeding complications. In patients with acute coronary syndrome without ST-segment elevation in EKG and in patients after percutaneous coronary intervention, a combination of ASA with clopidogrel should be preferred after consideration of the risk of major bleeding. In patients after acute myocardial infarction the ASA is less effective than oral anticoagulants, however it may be used in combination with a low-dose warfarin or alone in patients with a higher risk of bleeding complications.

After stroke or TIA, ASA should be preferred instead of high-dosed anticoagulants due to fewer bleeding complications. Especially in these patients the combination of ASA with dipyridamole retard is advantageous with regard to the rate of recurrent strokes over ASA administration alone.

The use of TAI, especially of low-dosed ASA in secondary prevention and also for some indications in primary prevention, may be cost-saving both from the statutory health insurance as well as from the societal perspective. This conclusion is limited as it is based on one pharmacoeconomic study only. The clinical input data of this study, which were generated in a meta-analysis, however possess a high degree of validity.


Wissenschaftliche Kurzfassung

1. Wissenschaftlicher Hintergrund

Der ischämische Schlaganfall (IS) - eine akut auftretende, fokale neurologische Symptomatik - wird durch eine unzureichende Durchblutung des Gehirns infolge eines zumindest passageren arteriellen Gefäßverschlusses verursacht und ist eine der häufigsten Todesursachen in Deutschland. Schätzungsweise ereignen sich in Deutschland jährlich 255 Schlaganfälle je 100.000 Einwohner.

Es werden außer nicht-medikamentösen auch verschiedene medikamentöse Maßnahmen zur Vorbeugung des erstmaligen oder wiederholten IS eingesetzt (Primär- bzw. Sekundärprävention des IS), darunter die Anwendung von Thrombozytenaggregationshemmern (bzw. Thrombozytenfunktionshemmern, TAH).

TAH sind Substanzen, deren Hauptwirkungsmechanismus darin besteht, den Kontakt von Thrombozyten mit dem Gefäßendothel, mit anderen Zellen sowie mit weiteren Thrombozyten zu verhindern. Mit diesem Mechanismus sind auch die Hauptnebenwirkungen dieser Substanzen verbunden: Blutungen intra- und extrakranieller Gefäße. Für die "klassische Prävention mit TAH" (gemeint ist eine langfristige Medikamenteneinnahme) sind überwiegend orale TAH von Bedeutung. Tagestherapiekosten belaufen sich für die wichtigsten orale TAH (ASS, Ticlopidin, Clopidogrel und Kombination ASS/Dipyridamol) auf 0,03 € (ASS) bis 2,30 € (Ticlopidin).

2. Forschungsfragen

Die medizinische Bewertung stellte sich die Fragen nach der medizinischen Effektivität des Einsatzes von TAH in der Prävention des IS im Vergleich zum alleinigen Management von Risikofaktoren sowie im Vergleich zu Antikoagulantien.

Aus gesundheitsökonomischer Sicht ist die Kosteneffektivität von TAH im Rahmen der Primär- und Sekundärprävention des IS sowie deren Einfluss auf die Lebensqualität von besonderer Bedeutung.

3. Medizinische Bewertung

3.1 Methodik

Die Literaturrecherche fand im Dezember 2003 statt und wurde in elektronischen medizinischen Datenbanken ausgeführt. Die Analyse wurde auf Basis von so genannten RCT (randomisierte kontrollierte Studien) und Metaanalysen von RCT durchgeführt. Die medizinische Bewertung geschah ohne externe Einflussnahme durch Industrie und Politik und es wurden in die Bewertung ausschließlich veröffentlichte Daten aufgenommen. In der Analyse wurden Endpunkte zu verschiedenen Typen von Schlaganfällen, zu Blutungen sowie ein kombinierter Endpunkt "schwere vaskulare Ereignisse" (SVE; Tod oder Schlaganfall oder Myokardinfarkt) berücksichtigt.

Die Ergebnisse von mehreren Studien wurden hinsichtlich ihrer methodischen Qualität und Heterogenität überprüft und in den Metaanalysen zusammengefasst, es wurden Schätzwerte vermiedener bzw. zusätzlicher Ereignisse pro 1000 einer Therapie zugewiesenen Patienten auf dem 95%-Vertrauensbereich ausgerechnet.

3.2 Ergebnisse

Es wurden insgesamt Daten aus 184 RCT (vs. Placebo) und aus 22 RCT (vs. Antikoagulantien) in die medizinische Bewertung einbezogen.

Analyse zum Schwerpunkt TAH im Vergleich zum alleinigen Management von Risikofaktoren und im Vergleich untereinander

Sowohl bei der primären als auch bei der sekundären Prävention des IS wird die Rate IS bei der Verordnung von TAH im Vergleich zum alleinigen Management von Risikofaktoren relativ um etwa 20% bis 35% reduziert (absolute Reduktion 4,8% vs. 6,6%; p<0,00001). Diese Risikoreduktion geht mit einer relativen Reduktion der Rate aller SVE (Tod / Schlaganfall / Myokardinfarkt) um etwa 16% bis 26% (absolute Reduktion 10,0% vs. 12,4%; p<0,00001) und mit einer relativen Zunahme der Rate großer Blutungen um etwa 45% bis 93% (absolute Zunahme 1,6% vs. 0,9%; p<0,00001) einher, dabei können die berechneten Effektschätzer generell auf alle Patientenkategorien und -subkategorien übertragen werden. Die absolute Reduktion des IS und aller SVE wird insgesamt deutlich größer im Vergleich zu der absoluten Zunahme an großen Blutungen. In einer Subpopulation mit einem niedrigen Risiko für IS und für große vaskuläre Ereignisse wird die absolute Anzahl erwarteter vermiedener Ereignisse der Anzahl an zusätzlichen Blutungskomplikationen annähernd gleich.
Die vorliegende Analyse weist direkt (d.h. vs. Placebo) eine Wirksamkeit für folgende einzelne TAH nach: ASS (ab 75 mg/Tag), Dipyridamol, Ticlopidin, Picotamid, Trapidil, Cilostazol und für die Kombination von ASS mit Dipyridamol in Bezug auf die Reduktion der Rate aller SVE. Eine direkte Reduktion der Rate aller Schlaganfälle konnte nur für ASS (über 150 mg/Tag), Dipyridamol, für die Kombination von ASS mit Dipyridamol sowie für Cilostazol nachgewiesen werden.

Es konnte kein Vorteil irgendeiner ASS-Dosis im Vergleich zu den anderen Dosierungen nachgewiesen werden.

Es wurde der Vorteil von Clopidogrel (sowie von Thienopyridinen insgesamt) im Vergleich zu ASS in Bezug auf die Reduktion der Rate aller SVE bei Patienten nach einem thrombotischen Ereignis (IS, Myokardinfarkt oder PAVK), von Ridogrel im Bezug auf die Rate "aller Schlaganfälle" (bei Patienten nach Myokardinfarkt) sowie von Ticlopidin, Triflusal und Sulfinpyrazon (für Triflusal nur bei Patienten nach TIA oder IS) bezüglich der Rate großer Blutungen nachgewiesen.

Die Kombination von retardiertem Dipyridamol mit ASS reduziert die Rate aller Schlaganfälle im Vergleich zur alleinigen Gabe von ASS vor allem bei Patienten nach vorherigem Schlaganfall oder TIA. Es wurde ebenfalls ein Vorteil der zusätzlichen Gabe von Clopidogrel zu ASS in der Reduktion der Rate aller SVE bei Patienten nach akutem koronarem Syndrom ohne ST-Segment Hebung beim EKG oder bei Patienten im Rahmen einer perkutanen koronaren Angioplastie nachgewiesen, allerdings auf Kosten von zusätzlichen großen Blutungen. Bei der zusätzlichen Gabe von oralen GBIIb/IIIa-Antagonisten wie Orbofiban oder Lotrafiban zu ASS ist dagegen mit einer Zunahme der Rate großer Blutungen ohne Wirksamkeitsverbesserung zu rechnen.
Analyse zum Schwerpunkt TAH im Vergleich zum Einsatz von Antikoagulantien

Die Antikoagulantien zeigten eine bessere Wirksamkeit im Vergleich zu TAH bei Patienten mit Vorhofflimmern in Bezug auf die Rate der IS und bei Patienten nach akuten kardiovaskulären Ereignissen in Bezug auf die Rate aller SVE (zumindest für Warfarin im Vergleich zu ASS). Besonders ausgeprägt ist der Präventionseffekt bei Patienten mit Vorhofflimmern. Dagegen ist die Verordnung von ASS im Vergleich zu hoch dosierten Antikoagulantien sowohl nach einem akuten als auch nach einem vorherigen Schlaganfall oder TIA vor allem sicherer, was die Blutungsrate, die Rate hämorrhagischer Schlaganfälle und schließlich die Rate aller wiederholten Schlaganfälle anbelangt.

Bei Patienten mit Vorhofflimmern zeigte reine Warfarin-Antikoagulation (INR=2,3-2,4) deutliche Vorteile im Vergleich zur kombinierten niedrig dosierten Warfarin plus ASS Kombinationstherapie (INR=1,1-1,3) bezüglich der Rate der IS ohne eine signifikante Zunahme großer Blutungen. Bei Patienten nach akutem Myokardinfarkt erzielten kombinierte 75 bis 81 mg/Tag ASS plus Warfarin Regime (INR=2,2-2,4) ähnliche Ergebnisse wie die reine Warfarin-Antikoagulation (INR=2,8-3,2) aber im Vergleich zur alleinigen Gabe von ASS in höherer Dosierung (160 mg/Tag) eine signifikante Zunahme der Rate an großen Blutungen.
3.3 Diskussion

Trotz mehrerer möglicher Verzerrungen der Ergebnisse, konnte ein Einfluss der einzelnen Faktoren nicht eindeutig nachgewiesen werden und alle Studienergebnisse wurden als valide für die eigenen Populationen und Therapiemodifikationen angesehen (interne Validität). Die Übertragbarkeit der Ergebnisse aus den vorliegenden Studien auf andere Populationen und Technologiemodifikationen ist begrenzt (externe Validität).

Die meisten aktueller Publikationen analysieren nur einen geringen Anteil der in der vorliegenden Analyse insgesamt berücksichtigten Studien und liefern im Vergleich zu dem vorliegenden Bericht ziemlich ähnliche Resultate.

4. Ökonomische Bewertung

4.1 Methodik

Zur Beantwortung der Forschungsfragen wurde eine umfangreiche Literaturrecherche in medizinischen und gesundheitsökonomischen Datenbanken durchgeführt. Darüber hinaus wurden einschlägige gesundheitsökonomische Zeitschriften auf Studien zur Thematik des HTA durchgesehen. Der Suchzeitraum umfasste die Jahre 1997 bis 2003. Es wurden ebenfalls die Internetseiten von Fachgesellschaften nach Informationen zu der Thematik des HTA-Berichts gesichtet und mit Herstellern von TAH mit dem Ziel Kontakt aufgenommen, unveröffentlichte Literatur zu erhalten.

Gegenstand der Studien mussten Kosten-, Kosteneffektivitäts- oder Kostennutzwertaspekte der Primär- oder Sekundärprävention des IS sein. Publikationen die Akutbehandlung IS betreffend und die ausschließlich die Prävention von Myokardinfarkten behandeln, wurden nicht-berücksichtigt. Publikationen mussten in deutscher, englischer, französischer oder spanischer Sprache verfasst sein. Die Bewertung der identifizierten Studien erfolgte unter Berücksichtigung des 25 Punkte umfassenden Kriterienkatalogs zur methodischen Qualität gesundheitsökonomischer Studien. Die Synthese der Information erfolgt deskriptiv.

4.2 Ergebnisse

Es konnten insgesamt drei Studien identifiziert werden, die dem gesundheitsökonomischen Standard von HTA gerecht wurden. Bei weiteren vier Studien traf dieses nicht zu, wobei in erster Linie die pharmakoökonomische Modellierung als in hohem Maß intransparent zu bezeichnen war. Die identifizierten Studien befassten sich mit ASS sowie Clopidogrel und Dipyridamol/ASS in der Primär- und der Sekundärprophylaxe von IS.

Die Verwendung von ASS im Rahmen der Primärprävention erscheint basierend auf den identifizierten Studien nur für Hochrisikopatienten mit einem bereits erlittenen Herzinfarkt als kosteneffektiv. Aus gesellschaftlicher Sicht lassen sich bei Verwendung von ASS in dieser Risikogruppe Kosteneinsparungen realisieren. Die Verwendung von ASS in der Sekundärprävention von Schlaganfällen hingegen ist bei allen Patientengruppen, die keine Unverträglichkeit für diesen Wirkstoff aufweisen, mit Kosteneinsparungen verbunden. Einschränkungen erfährt diese Aussage allerdings dadurch, dass nur eine Studie identifiziert werden konnte, die die Prävention von Schlaganfällen zum Thema hat. Aus gesundheitsökonomischer Sicht ist somit die Erstellung noch weiterer Studien auch vor dem Hintergrund der gesundheitspolitischen Bedeutung von IS wünschenswert, damit die Ergebnisse erhärtet werden.

Die Verwendung von Clopidogrel ist als nicht kosteneffektiv einzustufen. Dieses ist darauf zurückzuführen, dass für eine bessere Wirksamkeit von Clopidogrel gegenüber ASS keine klinisch gesicherten Daten existieren. Bei deutlich höheren Kosten für diesen Wirkstoff verfügt ASS somit über ein günstigeres Kosteneffektivitätsverhältnis. Clopidogrel sollte daher vorwiegend bei Patienten angewendet werden, bei denen sich ASS als unwirksam erwiesen hat oder bei denen eine ASS-Unverträglichkeit vorliegt.

Der Einsatz von Kombination ASS/Dipyridamol in der Sekundärprävention des IS kann eine kosteneffektive Alternative zu ASS sein. Die Autoren ermittelten folgende Kosteneffektivitätsverhältnisse:

- £ 3200 (€ 4800)/Lebensjahr mit funktionellen Einschränkungen,
- £ 1800 (€ 2700)/Lebensjahr ohne funktionelle Einschränkungen,
- £ 1900/(€ 2900) pro vermiedener Schlaganfall,
- £ 600/(€ 900) schlaganfallfreies Lebensjahr.
4.3 Diskussion

Bei der Interpretation der ökonomischen Ergebnisse ist zu berücksichtigen, dass die einzelnen Modelle nicht für deutsche Verhältnisse konstruiert worden sind. Eine grundsätzliche Übertragbarkeit scheint aber gegeben, da die Kosten und Kostengerüste jeweils transparent dargestellt worden sind und die Einzelpreise größenmäßig den deutschen entsprechen. Pharmakoökonomische Studien für deutsche Verhältnisse sind nicht nur aufgrund der epidemiologischen Bedeutung von IS wünschenswert.

5. Zusammenfassende Diskussion aller Ergebnisse

Die exklusive Betrachtung der Schlaganfallprävention ist nur eingeschränkt hilfreich, da Ärzte bei der Verordnung von TAH von einer Vermeidung aller thrombotischen Ereignissen ausgehen. Bei den Präventionsmaßnahmen muss besonders die Bedeutung der nicht-medikamentösen Prävention hervorgehoben werden.

6. Schlussfolgerung

Aus medizinischer Sicht können TAH zur primären und sekundären Prävention des IS bei Patienten mit einem hohen Risiko für SVE und mit einem niedrigen Risiko für Blutungskomplikationen empfohlen werden. Dabei ist eine Reduktion aller SVE und eine Zunahme der Blutungen zu erwarten. Die absolute Reduktion thrombotischer Ereignisse wird deutlich größer im Vergleich zur absoluten Zunahme an Blutungskomplikationen. Bei den Patienten mit niedrigem Risiko für SVE (z.B. ohne vorherige Schlaganfälle und Myokardinfarkte) ist der zu erwartende Unterschied von vermiedenen Ereignissen in Relation zu den zusätzlichen Blutungen kleiner, was den Einsatz von TAH weniger sinnvoll bis kaum sinnvoll macht.

Grundsätzlich sollte ASS in einer Dosis über 75 mg/Tag verordnet werden. Es können auch weitere TAH wie Dipyridamol, Ticlopidin oder Clopidogrel und (wegen begrenzter Datenlage nur bei bestimmten Patientengruppen) Picotamid, Trapidil, Cilostazol, Triflusal, Ridogrel oder Sulfinpyrazon als Substitute sowie Dipyridamol oder Clopidogrel als Zusatzmedikamente zu ASS angewendet werden. Dabei ist der Einsatz von Ticlopidin, Sulfinpyrazon und Triflusal vorteilhafter im Vergleich zu ASS hinsichtlich der erwarteten Blutungskomplikationen. Clopidogrel hat auch einen Vorteil im Vergleich zu ASS bezüglich des Risikos aller SVE bei Patienten nach einem vorherigen thrombotischen Ereignis, ohne dass das Schlaganfallrisiko selbst wesentlich beeinflusst wird. Kombinationen von ASS mit oralen GPIIb/IIIa Antagonisten sollten wegen einer erhöhten Gefahr von Blutungskomplikationen nur bei Sonderindikationen verordnet werden.

Bei Patienten mit Vorhofflimmern ist ASS deutlich weniger wirksam als Antikoagulation und sollte eher nur im Fall eines erhöhten Risikos an Blutungskomplikationen verordnet werden. Bei Patienten nach akutem koronarem Syndrom ohne ST-Segment Hebung beim EKG und bei Patienten im Rahmen einer perkutanen koronaren Angioplastie sollte ASS in Kombination mit Clopidogrel nach Abwägung des Risikos für zusätzliche große Blutungen bevorzugt werden. Bei Patienten nach akutem Myokardinfarkt ist ASS weniger wirksam als orale Antikoagulantien, kann aber in Kombination mit Warfarin eingesetzt werden oder allein bei Patienten mit einem höheren Risiko an Blutungskomplikationen.

Nach einem Schlaganfall oder TIA sollte ASS statt hoch dosierten Antikoagulantien wegen selteneren Blutungskomplikationen bevorzugt werden. Vor allem bei Patienten nach einem Schlaganfall oder einer TIA hat die Kombination von ASS mit retardiertem Dipyridamol einen nachgewiesenen Vorteil im Vergleich zur alleinigen Gabe von ASS in Bezug auf die Rate wiederholter Schlaganfälle.

Die Verwendung TAH, namentlich von niedrig dosierter, ASS kann in der Sekundärprophylaxe und unter einer strengen Indikationsstellung auch in der Primärprophylaxe sowohl aus der Sicht der Kostenträger aber auch aus der Perspektive der Gesellschaft mit Kosteneinsparungen verbunden sein. Diese Aussage erfährt jedoch eine Einschränkung dadurch, dass diese lediglich auf einer pharmakoökonomischen Studie beruht. Die klinischen Inputdaten, die aus einer Metaanalyse generiert worden sind, verfügen jedoch auf hohes Maß an Validität.
