Clear cell petro-clival meningioma in a 5-year old child: case report

Methods: This case of a 5-year old girl that presented with a one-week history of cephalgia, ataxia and left sided torticollis. Prior to this, the girl was normally developed without any pathological signs. MR-imaging revealed a dumpbell-shaped homogenously Gadolinium-enhancing mass in the left cerebello-pontine angle and the left temporal fossa adjacent to the cavernous sinus and the major vessels of the anterior and posterior circulation. There was a massive displacement of the brainstem and mesencephalon.
Results: A staged operative approach was chosen. First, a suboccipital craniotomy was performed and the tumour was resected completely within the posterior fossa. All cranial nerves were pushed apart without a clear nerval or dural matrix. A second operation via a modified fronto-temporal craniotomy was performed three months later. Between operations, further tumour growth was observed. A complete removal of the tumour was achieved. The follow-up examinations over the past 4 years did not reveal any residual tumour or recurrence. The child recovered completely and is developing normally.
Conclusions: Clear cell meningiomas are classified as WHO °II meningiomas with an increased risk for recurrence. They represent a rare subtype that is more frequent in children. This case presents a rare tumour of extensive dimensions that is discussed with respect to molecular diagnostics.
