Introduction: authors report function sparing surgical treatment of early tumors (T1, T2) of the piriform sinus. Surgical technique, oncological and functional result of hemipharyngo-laryngectomy are demonstrated .

Method: 25 patients were retrospectively reviewed who underwent hemipharyngo-laryngectomy with neck dissection for hypopharyngeal-laryngeal squamous cell carcinoma and neck metastasis in our department. Our surgical method involved unilateral neck incision, removal of half of the hyoid bone, prelaryngeal muscles are unilateraly dissected and resection the lateral part of the thyroid cartilage, infiltrated pyriform sinus and part of the larynx. Perichondrium of the resected thyroid cartilage was preserved for reconstruction of the neopharynx beside the pharyngeal mucosa and prelaryngeal muscles. 12 patients got postoperative irradiation.

Results: Of the 25 patients 12 have no sign of recurrence (48%), 5 patients had late metastasis (20%) required sRND, 2 patients had local recurrance and required total laryngectomy. In 2 patients second primary tumor (8%) was observeed, 2 patients (8%) had distant metastasis (paratracheal-mediastinal, inguinal). One patient died of loco-regional recurrences and another one died of other disease.

Conclusion: hemipharyngo-laryngectomy represents a method preserving laryngeal function and providing acceptable swallowing in selective cases of early piriform sinus cancer.
