Interdisciplinary approach in rehabilitation after upper limbs prosthetics

Objectives: The following research aims to evaluate the effectiveness of a interdisciplinary medical, technical and rehabilitation approach in upper limb prosthetics of children.

Methods: 88 prosthetics of the upper limbs for children from 3 to 17 years old with congenital disorders have been performed from June to December 2018.

In the first group 44 children received age specific materials on expectations, physical and psychological preparation for the device receiving. Immediately after prosthetics they underwent 1 day intensive individual complex of rehabilitation based on a multidisciplinary approach (occupational therapy, kinesiotherapy,class with a clinical psychologist).

The second group of 44 children did not receive any age specific materials or rehabilitation neither prior, nor after prosthetics.

For the first group the success of the rehabilitation program was assessed with the test “9-Hole Peg Test”, conducted before the start of the rehabilitation program and after its completion. Also parents from both groups conducted a survey after 6 month of the prothesis usage.

Results and Conclusion: In allcases after 1 day intensive individual complex of rehabilitation there was a significant improvement in the results of the 9-Hole Peg Test:

  • reduction of the time required to complete the task by 70-85%;
  • increase of the movements accuracy - the number of errors and drops of pegs decreased by 2-2,6 times.

The results of the survey showed a significant difference between the prothesis usage in 2 groups.

Group 1 had more permanent pattern of usage: more than 80% of children wear the device daily, compared to 32% in the 2 group. Also the duration of usage per week was 2-3,5 times longer. Children from group 2 mostly did not wear the prothetic to the social events/school etc, while 90% of children from group 1 did. The average satisfaction with the device in group 1 was 8,7 compared to 6,5 in group 2.

Based on the data obtained so far, it can be assumed that even a short interdisciplinary rehabilitation program and age specific materials on expectations and preparations for the prosthetics can significantly improve the patient's skills with the prosthesis, pattern of the device usage and general satisfaction.

In the future we plan to expand the sample size to create a statistically reliable justification for the use of a interdisciplinary approach in the rehabilitation of this group of patients.
