Objective: This report should increase the awareness of a rare complication like re-expansion pulmonary edema (REPE) and elucidate potential risk factors.

Methods: Report of a case and review of literature

Results: Due to the rare but potential lethal complication of REPE especially after draining a pneumothorax or pleural effusion, the knowledge of this complication, its risk factors (young age, duration of collapse, rapid expansion of the lung) as well as prophylactic actions are important for clinical practice.

Conclusion: However, preventive measures may not avoid the development of REPE, awareness of this complication leads to immediate necessary diagnostics and treatment.
