Objective: To investigate the factors and consequences of non-convulsive status epilepticus (NCSE) in patients with spontaneous subarachnoid hemorrhage (SAH).
Methods: Patients with SAH treated at the author’s institution between 10/2012 and 01/2018 were included in a prospectively conducted database. Clinical, radiological, and electrophysiological data were collected and further analyzed.
Results: 533 patients with SAH were treated. Altogether 75 patients (14%) had epileptic seizures. 16 of these 75 patients (21%) developed NCSE. In summary, 2 patients (12,5%) presented with generalized NCSE and died before hospital discharge and 14 patients (87,5%) had focal NCSE. In the group of patients with focal NCSE, 13 (93%) had an unfavorable outcome and one patient (7%) achieved a favorable outcome at 6 months.
At 6 months, 10 (63%) patients were dead, 5 (31%) achieved an unfavorable, and one patient (6%) achieved a favorable outcome. The multivariate regression analysis revealed that age greater than 65 years was an independent predictor for the development of NCSE (p=0.001).
Conclusion: NCSE is rare after SAH but associated with high morbidity and mortality. Age seems to be an independent predictor for the development of NCSE.
