Background: According to the social theory of organized medical treatment from Vogd [1] every profession has a dichotomes objective. In the medical field, for example, this objective is whether someone is sick or healthy. But what is the objective of midwifery care? This question has not yet been answered precisely by midwifery science.

Aim/Research Question: Describe the objective and aims of midwifery in a hierarchy model and how it can be applied in education, practice, and for the professional role within the health system.

Methods: The developed model is based on literature on concepts, theories and frameworks of midwifery as well as on literature on the experiences and preferences of women with midwives. For the development, the method of theory development according to Walker and Avant [2] was used, along the questions “what is the purpose of this objective?” and “how is this objective achieved?”. Development was done in 2018/2019. A vote on ethics was not necessary for the present research design.

Results: The model shows on three levels the prerequisites for achieving high-quality midwife care and at the same time it shows why we midwives act, how we act.

Relevance: It will be shown how the model can be applied in everyday practice, education, research and for the professional role within the health system. For example, the role of relationship building between woman and midwife can be made explicit using the model. This can improve interprofessional cooperation and understanding of the work within the public health system.

Recommendations/Conclusion: Theoretical models structure our thinking and argumentation. This is indispensable for practice, teaching and professional positioning in healthcare. The theoretical work requires constant further development and application.

Ethics and conflicts of interest: No vote on ethics was obtained. The research was financed by own resources. There are no conflicts of interest.
