Do gastric bypass operations alter bone quality? Biomechanical investigations in a rat model

Objectives: Adipositas is becoming an emerging health and economic problem. Dieting and medication therapy are unsatisfying with high recidivism rates. The only treatment with positive long-term results is bariatric operative treatment. Among bariatric methods the Roux-en-Y-gastric bypass (RYGB) is the gold standard with a positive benefit-risk profile. There is some data that RYGB-operations may have an negative impact on bone quality. Currently, there is no relevant data about a reduced bone mineral density (BMD) and a higher fracture risk after bariatric bypass operations. Therefore we asked if there is a long-term influence after gastric bypass operations on bone quality in a rat model.
Methods: 18 male Wistar rats were randomised and divided into two equal groups. 9 rats underwent a RYGB-operation and the other 9 rats underwent a sham-operation. 200 days post surgery animals were sacrificed and the tibiae and femora were explanted for further biomechanical and radiological investigations.
Results and Conclusion: The quality of bone of RYGB-operated rats was significant inferior in strength and stiffness compared to the sham-group. Besides, microcomputertomography showed a significant lower BMD in RYGB-operated rats.
The often performed gastric bypass surgery (RYGB) has a negative impact on the bone quality.
