Introduction: According to ASAS, extra-articular (uveitis, psoriasis, inflammatory bowel diseases) and non-axial (arthritis, dactylitis, enthesitis) manifestations refer to the classification criteria for axial spondyloarthritis (axSpA). However, currently there are few studies evaluating the dynamics of extra-articular and non-axial manifestations in patients with early axSpA.

Objective: to evaluate the dynamics of extra-articular and non-axial manifestations in patients with early axSpA managed according to the T2T strategy during 3 years of active monitoring.

Methods: Currently the CORSAR cohort includes 175 patients with early axSpA (ASAS criteria, 2009) with inflammatory back pain duration < 5 years. The analysis included 56 patients who were followed for at least 3 years. Patients were examined in accordance with ASAS recommendations at inclusion and after 3 years, of whom there were 30 (53.5%) men and 26 (46.5%) women, p>0.05. Patients’ mean age was 27.8 (5.8) years, and mean disease duration was 21.2 (16.2) months. At baseline there were 31 (55.3%) AS patients (according to the modified New York criteria 1984), and 25 (44.7%) patients with nonradiographic axSpA (nr-axSpA). 53 (94.6%) patients were positive for HLA B27. All patients were followed and managed according to treat-to-target (T2T) strategy.

Results: At baseline patients with early axSpA had significantly higher counts for such non-axial manifestations as arthritis, dactylitis, and enteritis, than after 3 years of follow-up (Table 1 [Tab. 1]). As for the extra-articular manifestations, the number of patients with a history of uveitis did not change during the follow-up period, psoriasis was documented at baseline and in 3 years in one patient only, and no patients had inflammatory bowel disease (Table 1 [Tab. 1]).

Conclusion: Enthesitis and arthritis were the most common non-axial manifestations of the early axSpA, which significantly decreased after 3 years of active monitoring being managed according to the T2T strategy. No new cases of extra-articular axSpA manifestations were registered during 3 years of FUP.
