Background: In diabetic patients with progressive proliferative retinopathy and macular edema a pars plana vitrectomy is necessary.
Materials and methods: In vitrectomies in diabetic eyes, there are often massive proliferative changes in both the retina and the iris. Intraoperative and postoperative bleeding are common.
Results: The intravitreal administration of anti-VEGF few days before the PPV reduces the congestion of the retinal vasoproliferationen and the iris. This makes it easy to remove membranes en bloc. A air-endotamponade further reduces the postoperative bleeding.
Conclusion: In patients with impaired Iristrophik and barrier disorders like diabetes the surgical procedure can be greatly simplified by the use of anti-VEGF. A hämostypische tamponade with silicone oil is usually not required and macular edema is treated with.
