OCT documentation of functioning filtering blebs: 1-year results

Objective
Evaluation of documentation of long-term functioning filtering blebs in optical coherence tomography (OCT) in their postoperative course of one year.
Methods
In 23 eyes of 22 patients a slit-lamp adapted 1310nm OCT of the filtering bleb was performed prospectively on postoperative day 3, 7, 21, 30 and after 3, 6 and 12 months following goniotrephination (with mitomycin c in 20 patients). Patients with high IOP and scarring of the filtering bleb were excluded.
Results
In the postoperative course 8 patients required needling of the filtering bleb (1-2x) and injection of 5-fluoruracil (3-8x). The IOP of all patients was within normal range without medication, medium was 10,1mmHg (6-15mmHg). In the OCT the functioning filtering blebs could be differentiated in 3 groups: group 1: 7 patients showed a cystic conjunctival configuration without subconjunctival space, group 2: 5 eyes of 4 patients had a bullous filtering bleb formation with massive subconjunctival space, group 3: 11 patients showed a mixture with moderate conjunctival thickening and moderate subconjunctival space. All functioning filtering blebs were associated with a low internal reflectivity in the OCT.
Conclusions
Long-term functioning filtering blebs can appear in different forms in their postoperative course. In OCT the functioning filtering blebs were categorized into 3 groups: cystic, bullous and mixed type. All filtering blebs with regulated IOP showed a low internal reflectivity in OCT.
