Objective

For preoperative antisepsis in cataract surgery the conjuctival fornices are routinely irrigated with PVP-iodine. We intended to establish whether there iodine penetrates into the anterior chamber after the use of PVP-iodine 1,25% and tested to 2 kinds of PVP-iodine 1,25% application schemes.

Methods

PVP-iodine 1,25% was applied for 1 minute to the conjunctival fornices prior to routine phacoemulsification cataract surgery. Two minutes later an aqueous humour sample was taken (=group A) and the iodine concentrations analysed and compared with samples from patients who in addition had received three drops of PVP-iodine 1,25% 1.5h prior to the conjunctival irrigation (=group B).

Results

After an intraoperative application of PVP-iodine 1,25% the concentration of iodine was 39,4±22,3μl/dl (10,1 to 86,2μg/dl, group A) and 94±76μl/dl in group B (21,6 to 199μg/dl). The difference was not statistically significant different (p=0,16).

Conclusions

The preoperative application of PVP-iodine 1,25% in addition to PVP-iodine irrigation of the conjuctiva results in a higher iodine concentration in the anterior chamber. The dose of iodine is however very low in all anterior chamber samples and within the range of normal renal iodine elimination without iodine supplementation. This iodine concentration in the aqueous humour is not expected to have any local toxic effect. Therefore the additional, preoperative PVP-iodine 1,25% application for a better antiseptic is safe alternative approach.
