Hintergrund: Genetische Polymorphismen in Genen, die für Proteine des Komplementsystems kodieren, zeigen eine Korrelation mit dem Vorliegen einer altersabhängigen Makuladegeneration (AMD). Die Auswirkung solcher Polymorphismen auf die systemische Komplementaktivierung wurde bisher nicht untersucht.

Methode: Die Plasmakonzentrationen von folgenden Proteinen des Komplementsystems wurden bei AMD Patienten (n=112) und Kontrollpersonen (n=67) bestimmt: Substrate, C3, C4 und Faktor B (FB), Marker für eine akute Komplementaktivierung, C3a und C5a, Marker für eine chronische Komplementaktivierung, actC3, Ba und C5b-9, Regulatoren des Komplementsystems Complement-Faktor H (CFH) und Faktor D (FD).

Ergebnisse: Die mediane Plasma-Konzentration des Regulators CFH sowie der Substrate C3 und C4 zeigte keinen Unterschied zwischen AMD-Patienten und Kontrollpersonen. Bei AMD-Patienten fand sich eine erhöhte Plasma-Konzentration des Substrats FB (AMD: 803 µg/ml, Kontrollen: 642 µg/ml; p=0.02) und des Regulators FD (AMD: 1.27 µg/ml; Kontrollen: 0.95 µg/ml; p < 0.001). Weiterhin waren alle Proteinmarker, die eine akute bzw. chronische Komplementaktivierung anzeigen, in der AMD-Gruppe signifikant erhöht: C3a (AMD: 15.5 ng/ml, Konrollen: 14.3 ng/ml; p=0.03), C5a (AMD: 1.85 ng/ml, Kontrollen: 1.67 ng/ml; p=0.04), C3d (AMD: 55.2 ng/ml, Kontrollen: 46.9 ng/ml; p<0.001), Ba (AMD: 1.33 µg/ml, Kontrollen: 1.09 µg/ml; p<0.001) und SC5b-9 (AMD: 188 units, Kontrollen: 159 units; p=0.01).

Schlussfolgerung: Bei AMD-Patienten findet sich eine systemische Konzentrationserhöhung von Proteinen der alternativen Komplementkaskade. Die stärksten Effekte wurden für die Proteine Faktor D, Ba und C3d gefunden.
