Considering the poor standard of performance in the presentation of ENT-Medicine in the G-DRG-flat rate system, the German association for ENT-Medicine (DGHNOKHC) and the ENT Medical Professional Association (Berufsverband), in cooperation with the DRG-Research Group of the University Hospital of Muenster, have carried out the DRG-Evaluation Project for ENT-Medicine. Concomitant with this in the G-DRG System 2005 on the basis of recommendations for change made by the body of experts, ENT surgical oncology was revised and fully restructured.

The aim of the investigation was to examine the presentation of ENT surgical oncology, in material and performance-appropriate terms,in the modified G-DRG system 2005.

Method: Retrospective analysis of the official DRG-data records (§21 KHEntgG) of 85,971 cases from 2003 from 40 ENT institutions (14 university hospitals, 16 urban hospitals and 10 other institutions). Prospective collection of data of 25,006 cases, including defined expenditure data within a period of four months in 2004. An analysis on the basis of 404 prospectively collected data records with surgically treated malignant tumours that was grouped into the adjacent DRG D02 of the G-DRG-System 2004 was performed.

Results: It was possible to improve the illustration of ENT surgical oncology in comparison to earlier versions in the G-DRG-System 2005. Nevertheless, there is a need for further adaptation as, based on the prospective random sample, case constellations were identified which were not illustrated appropriately either materially or in relation to performance. It was possible to improve this by targeted modifications of the system of classification.

The recommendations for adaptation based on the results of the evaluation project for the further development of ENT surgical oncology in the G-DRG-System are presented.
