Different effects of sibutramin on metabolic parameters depending on different antihypertensive treatments

Weight reduction lead generally to a substancial improvement of the carbohydrate and lipid metabolism. This is also seen by the serotonin- and norepinephrine reuptake inhibitor sibutramine (sib). On the other hand it is unknown if various antihypertensive combination regimes can influence these effects.
In a randomizec, doubleblind multicenter study with 169 obese hypertensive patients we investigated the sib (10/15 mg/d) vs placebo effects on metabolic parameters in 3 groups treated by 3 different antihypertensive regimes (A: ramipril plus felodipine ER 5/5 mg, B: trandolapril plus verapamil SR 2/180 mg, C: metoprolol plus HCT 95/12.5 mg).
The weight reduction was significant higher in the groups A and B, treated with ACEI/calciumantagonist combinations (-6.7, -6.0 %) compared to group C treated with betablocker and diuretic (-3.3 %). The effects of sib on metabolic parameters in these different groups are listed in Table 1 [Tab. 1].
Conclusion: Compared to ACEI/calciumantagonist combinations the betablocker/diuretic combination showed unfavourable effects especially in regard to the carbohydrate metabolism and weight reduction in obese hypertensive patients treated with sib.
