Introduction: Granular cell tumors (Abrikossof tumors, granular cell schwannomas) are benign tumors of the nervous system and contain lipoproteid granules in the cells. They are frequently found in the striated musculature of the head and neck region (tongue, oesophagus). The larynx is affected in approximately 6% of all locations described.

Method: A 30-year-old women with hoarseness and the laryngoscopic image of a vocal fold polyp in the anterior part of the right vocal fold and of a smooth swelling of the left vocal fold was introduced for phonosurgery. The histology of the resected polyp revealed a granular cell tumor. The MRI described a 8 x 4 x 4 mm tumor of the left vocal fold. A partial chordectomy of the left vocal fold was carried out.

Discussion: The malignancy rate of all granular cell tumors is about 1-2%. The pseudoinvasive growth of the tumors into the adjacent musculature is no malign expansion, but indicates an orientation of the tumor growth along the muscles. The treatment is depended on the histology, the extension of the tumor and the symptoms of the patient. The normal treatment is surgical.

Conclusions: The granular cell tumor is a rare differential diagnosis of benign changes of the larynx..
