Intraindividual comparison of postoperative total ocular aberrations and contrast sensitivity after implantation of monofocal IOL’s with spherical and prolate surface

Objective
Total ocular aberrations are increasing with age. The greatest amount are lens induced spherical aberrations. Because of implantation of classical IOL's the spherical aberrations increasing. This can be one reason for the reduced contrast sensitivity and night driving problems of pseudophakic patients. These postoperative effects should be reduced after implantation of IOL's with prolate surface.
Methods
To investigate the positive effects according to the reduction of spherical aberrations we implanted randomised in 25 patients in one eye an IOL with spherical surface (CeeOnEdge™) and in the other eye an IOL with prolate surface (Tecnis™). 3 and 6 months postoperative contrast sensitivity and total ocular wavefront were analyzed and intraindividual compared.
Results
In all patients spherical aberrations were significant lower in the eyes with Tecnis™ lens than in the control eyes with CeeOnEdge™ lens implantation. The intraindividual difference in contrast sensitivity and its interindividual comparison displayed an improved contrast sensitivity under mesopic conditions after implantation of Tecnis™-IOL.
Conclusions
Implantation of foldable silicon lenses with prolate anterior surface reduces total ocular spherical aberrations and increases the contrast sensitivity in most patients. This effect shows great interindividual variability because of the various influences on the contrast sensitivity.
