Objective

In context to small incision techniques the claim for the refractive outcome in cataract surgery grew during the last years. Uncorrected visual acuity should be preferably perfect after surgery. With toric intraocular lenses in principle it is possible for patients with astigmatism too.

Methods

In 21 eyes of 17 patients with corneal astigmatism of 1.5 to 3.75 D phacoemulsification was performed via a 4 mm sclerocorneal tunnel incision in the steep meridian. A toric silicone lens with Z-haptic was implanted in the bag (Typ MS 6117 TU, Dr. Schmidt). The Follow-up was 6 weeks, uncorrected and corrected vi­sual acuity, corneal and total astigmatism were evaluated.

Results

The mean uncorrected visual acuity before surgery was 0.13±0.11, which was improved to 0.71±0.23; mean best corrected visual acuity before surgery was 0.34±0.2, and 0.89±0.19 after surgery. The mean total astigmatism was 2,4±1,03 D and was reduced to 0.5±0.48 D after surgery. Mean corneal astigmatism before surgery was 2.3±0.71 D and decreased to 1.6±0.71 D after surgery. The mean rotation of the toric lenses was 2.3° (range 0°-12°) on the first postoperative day and 3.4° (range 0°-12°) after 6 weeks.

Conclusions

Implantation of toric silicone lenses with newer haptics improves the uncorrected visual acuity in patients with corneal astigmatism up to 3.75 dpt.. Total astigmatism was considerably reduced. Reliable keratometry just as rotational stability of the implanted lenses is important especially for the uncorrected visual acuity.
