The incidence of complications after corneal refractive surgery (keratotomies and Excimer-laser) are between 0.5% to 2%. Using modern microkeratomes of the newest generation cutting problems (except epithelial defects) are almost not seen anymore. The most important complications like epithelial ingrowth, striae and folds and diffuse lamellar keratitis are demonstrated and discussed. Reoperations (most of the cases because of an undercorrection) should be performed with wavefront guided systems. Keratectasias should be recognized early and treated immidiarely by collagen cross-linking using riboflavin and UV-light.

Complications after phacic lens implantation are endothelial cell loss, pupil ovalisation, anterior synechia, secondary glaucoma and anterior subcapsular cataract. All of these are discussed
