Complex processes of the vitreoretinal interface may cause retinal detachment. They require specific therapeutic strategies. During the past 50 years, increasingly effective concepts have been developed leading from simple diathermy coagulation to indenting procedures and vitreoretinal surgery. The invention of pars plana vitrectomy (PPV) makes a direct microsurgical manipulation possible at the vitreoretinal interface. A preretinal application of therapeutic drugs will be available in the near future.
The indication for a certain surgical procedure should be based on pathophysiological reasoning, evidence from controlled clinical studies and permanent critical judgment of pre- and postoperative results. This leads to a pathology based rather than schematic planning of individual surgical procedures.
Some surgical concepts and their results will be illustrated on the basis of typical situations like PVR and diabetic and non-diabetic vitreoretinal disorders.
