Objective

To present diagnostic difficulties of glaucoma in the optic disc coloboma cases.

Methods

Authors present three patients (2 women: 25 and 49 years old, 1 man: 50-years-old), who have been treated because of glaucoma and have been sent to Department of Ophthalmology in Poznań for the further treatment. Whole ophthalmological examination including pachymetry with Specular Microscope Topcon 2000 and optic disc analysis with RTA or HRT has been done.

Results

Three cases with diagnosed optic disc coloboma: one woman with bilateral disc coloboma with chorioretinal involvement, macular ectopia, microphthalmia and retrobulbar cyst; one patient with disc coloboma in one eye and microcornea and a man with partial fissure of the optic disc without any other abnormalities. Glaucoma has been excluded in two cases and they were left without medication. One patient required glaucoma surgery, because of none response to pharmacological treatment.

Conclusions

The diagnosis of glaucoma in colobomatous patients provides a lot of diagnostic difficulties. However the typical fissure of the optic disc and hyperpigmentation located in the inferonasal quadrant, the size of the disc should make us suspect the disc coloboma. In such cases the exclusion of the glaucoma is mostly possible after a very detailed examination involving modern investigation methods.
