Vitrectomy is gaining popularity among the treatment modalities for diffuse diabetic macular edema (DME). Recent case series and retrospective studies reported promising results.
However, most of the published studies presented very small and heterogenous patient groups as well as differing treatment modalities. Our systematic review of the literature did not find one randomized controlled trial which could proove the effectiveness of vitrectomy for DME. Against this background, we report the results of our own prospective randomized trial that investigated the question whether in patients with DME a posterior vitreous detachment (PVD) alone or with an additional removal of the inner limiting membrane (ILM) is advantageous. The evidence arising from these new data and the results of the published studies are critically discussed.
