Objective: To assess morphological differences of vessels with and without An.

Methods: 65 ICAs were reconstructed from 3D angiographies of the anterior circulation: 5 without any An, 25 with ICA An, 35 with An distal to the ICA. Diameter and curvature were evaluated statistically in relation to the presence of An. Hemodynamic computational simulations calculated wall shear stress (WSS) on some samples from each geometrical-statistical vessel class.

Results: Two ICA categories were identified: i) larger diameter and narrow bends characterized ICAs with An and also those without An ii) smaller diameter and more straight bends characterized ICAs of vessel trees where the An was located distal to the ICA. In ICAs with An, the WSS peaks were higher, particularly distributed on the internal side of the distal siphon curve and on the upper portion of the most distal ICA tract. In these examples the influence of siphon curve morphology on WSS pattern in the distal ICA tract was evident. The An itself also determines increased WSS levels on the inflow and outflow regions of the parent vessel and on eventual side vessels.

Conclusions: These initial analyses indicate the importance of the geometric features of the parent vessel for the hemodynamic impact on the wall, crucial in degenerative diseases like An and atherosclerotic plaques. If the more straight shape of the intracranial ICA in patients with An distal to the ICA is to relate to a hemodynamic problem of siphon inefficiency still needs to be determined.
