Isolated tongue tonsillectomy for OSAS

Retrospective study: 12 patients with OSAS got isolated tongue tonsillectomy: 4 mild OSAS, 4 moderate OSAS and 4 severe OSAS.

The patients had previously sleep recording, MRI and preoperative sleep endoscopy (DISE), BMI was less than 30. Mean age was 44 years. 3 tonsillectomy techniques were used: coblation, diode laser and robot.

Postop apnea/hypopnea index decreases from 28.4 to 11.7. Postop Epworth from 12.5 to 7.2. Snoring decreases by 83%. BMI decreased from 28.6 to 27.4. For severe OSAS Group, apnea/hyponea index decreased from 50 to18. Patients group with complete isolated tongue base obstruction were all cured. When tongue obstruction is partial or associated with a second obstructive site, surgery was less effective.

Lingual tonsillectomy can cure severe OSAS if DISE shows a unique and comprete tonsil obstructive site.
