Summary: Inner ear extension of schwannomas is an extremely rare finding, which can cause hearing loss, Tinnitus and Vertigo.
We report on a patient, who introduced itself with a progressive hearing reduction, as well as Tinnitus on the right ear. Under the rheologic infusion therapy with cortison accomplished in another place there was no improvement of the hearing.
In the audiogram we find a complete hearing loss of the side concerned, the vestibulogramm impressed a complete loss of the rightside labyrinth. In the MRI a structure appeared in ranges of the basal cochlea with clearly enhancement. From the MRI aspect it is concerned to be a intracochlear schwannoma. As a differential diagnosis a chronically granulating inflammation is also possible. We decided for half-year control clinically and via MRI; these resulted in so far no further growth of the structure.
Conclusion: Intralabyrinthine schwannomas are rare tumors that mimic the clinical features of many other neurotologic conditions. A precise imaging via MRI is required to detect these tumors. Surgical treatment indicated for specific indications and will be needed in the minority of patients with this disorder. Observation is an appropriate option for patients who have serviceable hearing.
