Objective

Studies in the 90s demonstrated that bilateral pseudophakic patients with monofocal IOLs rarely regained full binocular vision. Multifocal IOLs achieved better results regarding stereopsis in these trials. The reason for this could be the Stiles Crawford effect. The intention of the actual trial was to investigate the effect of a monofocal aspheric lens with known contrast sensitivity improvement on stereoacuity in patients bilaterally implanted.

Methods

Strabology was performed in 55 pseudophakic patients (110 eyes) before surgery, and at 4 days and 6 weeks post-operatively.

Results

In contrast to earlier assumptions in all patients with the principal ability of stereopsis binocular vision could be restored completely by implantation of the aspheric IOL. Reasons for the few cases (7/55), where patients did not achieve stereopsis were IOL-independent (e.g. micro-strabism, head nodding due to Parkinson's disease, glaucoma).

Conclusions

Our results proved that aspheric IOL can restore stereoacuity in cataract patients, if implanted bilateral. Studies on the percentage of patients reaching stereoacuity after bilateral cataract surgery should be performed for all available lens types using standardized test methods. Major issues might be inclusion and exclusion criteria.
