Objective

Foldable IOLs containing different material (silicon, acrylic, hydrogel) are available since one decade. Several studies showed a postoperative visual acuity equal to rigid intraocular lenses. However, the photopic visual acuity is only one aspect of the broad visual spectrum. Although many patients have an excellent visual acuity, some suffer from increased glare and reduced mesoptic visual acuity. The additional evaluation of visual acuity under mesoptic and glare conditions allows therefore additional functional testing which compared more to a real life condition. In this pilot study we investigated intraocular scattered light in relation to the edge design of the IOL (sharp vs. round edge) by means of contrast sensivity and glare.

Methods

Patients with monofocal silikon-foldable lenses on both eyes were included. On one side patients received a rectangle shaped optic edge lens (CeeOn Edge 911A - Pharmacia GmbH) and at the other side a round shaped lens (AMO Phacoflex SI30NB - Allergan). All monolateral tests (Pelli-Robson, Vistech, Mesoptometer) were performed 3 month post surgery (±2 weeks).

Results

The age of the patients (10 female, 6 male) was between 62 and 82 years (mean 77). The average postoperative visual acuity with both lenses was 0,95 cum correctione. The Pelli-Robson-test (1m, 120cd/m2 with and without glare) and the Vistech-Test (3m, 120cd/m2) revealed no significant differences in contrast sensitivity between both lens types. On average, the mesopic vision (Mesoptometer) was for 0,1 cd/m2 without glare and for 0,03 cd/m2 one contrast line better for the eye with the SI30-lens. Only 4 patients were able to see the contrast pattern for 0,1 cd/m2 with glare.

Conclusions

This pilot study revealed in 16 patients a minor increased contrast sensivity for patients with round shaped designed IOLs. The mesoptic vision with glare was low.
