Evaluation of optical quality of extended depth of focus intraocular lenses in polychromatic light

Purpose: Cataract surgery with new Extended Depth of Focus (EDoF) technology aims for high-quality vision and spectacle independence. Unlike monofocal and multifocal intraocular lenses (IOL), EDoF IOLs can provide clear vision at different distances, while reducing visual side effects. In this study, the optical quality of EDoF IOLs (Tecnis Symfony (Abbott Medical Optics)) was evaluated using international standard test method in order to evaluate their optical performance. The IOLs were examined under a polychromatic light condition, which mimics the real world environment.
Methods: Five Symfony IOLs with the same refractive power of 20.00 diopters were measured using OptiSpheric® IOL PRO 2 (TRIOPTICS)-fully automated optical test device. The powers of five lenses were measured. A model eye was set, containing a spherical-aberrated model cornea, a photopic-eye-response filter and a white light source. Modulation transfer function (MTF) areas were measured for different aperture sizes and ranges of defocus to determine optical quality.
Results: The measured mean power of the five Symfony IOLs was 20.05±0.05D. The MTF area values at range of defocus of -2.0D, -1.5D, -1.0D, -0.5D, 0D, +0.5D were 0.48, 0.50, 0.38, 0.41, 0.48 and 0.38, respectively. The MTF area values at pupil sizes of 2mm, 3mm, 4mm and 4.5mm at far focus were 0.52, 0.48, 0.47, 0.47 and at the best near focus 0.59, 0.52, 0.45, 0.43, respectively. The United States Air Force resolution test charts at ranges of defocus were provided to confirm optical quality.
Conclusions: The measured powers showed low variability (SD=0.05D) between the five Symfony IOLs. We demonstrated a good optical quality of the Symfony lenses under the polychromatic light condition. The optical performance was comparable at the far and near focus and for all pupil sizes.
