Minimally invasive scleral fixation of the intraocular lens using suture thread inserter combined with 25-Gauge vitrectomy

Purpose: To describe the surgical technique of the scleral fixation of the intraocular lens (IOL) using suture thread inserter which is the new designed device combi ned with 25-gauge vitrectomy.
Methods: Scleral fixation of the IOL was performed in five patients with aphakia using a new technique combined with vitrectomy. Before 3 port insertion to the sclera, conjunctival peritomy and scleral tunnels were made at 2 and 8 o’clock. Infusion cannula was inserted through the inferotemporal sclera and two cannula s were inserted through the scleral tunnels 1.5 or 2.0 mm posterior to the corneal limbus at 2- and 8-o’clock. After the vitrectomy, the looped part of the 10–0 polypropylene suture (PC-9: Alcon USA) was inserted at 2- and 8-o’clock port with suture thread inserter. The haptics of the foldable IOL were sutured by cowhitch method. After removing the infusion cannulas, 10–0 polypropylene were tied to scleral tunnels.
Results: The IOL was fixed stably and there were no intraoperative and postoperative complications.
Conclusions: This new technique was safe and minimam invasive for using 2 ports both as vit rectomy port and scleral IOL suture part. Using thread inserter for intraocular harness cord, there are no risk of the hemmorrhage compared with using needle directly.
