Background and state of (inter)national research: Chronic heart diseases (CHD) are some of the most frequent causes of morbidity and mortality in Germany. The etiologies of these diseases are diverse; however, they are becoming increasingly more common due to demographic changes [1]. Accordingly, the health care system must also provide comprehensive and needs-based outpatient care for this growing number of patients in the medium and long term. This review will investigate models of care for patients with CHD in outpatient care (OC) to generate an overview of existing evidence.

Questions and objectives: Primary review objectives:

1.
Review the literature systematically to determine models of care in outpatient care settings.
2.
To map the evidence found by the review and present this visually.
3.
An overview of reviews of interventions for patients with CHD in OC.

Method: A Mapping Review and Overview of Systematic Reviews conduct based on a broad, systematic literature search in the databases PubMed, Cochrane Library, Web of Science and Scopus. The search strategy was based on four concepts: terms related to CHD (coronary heart disease, heart failure and atrial fibrillation), models of care, OC and *reviews. The scope of interventions considered will be broad, encompassing all models of care, independent of age, ethnicity, social status, and sexes. Interventions for specific vulnerable groups (e.g. pregnant woman and palliative care) and infants was excluded. Single and multicomponent interventions (human and technology based) were included.

Results: The search yielded 17428 results from the database search. After removing the duplicates, 6643 articles were screened by title/abstract for inclusion. We included 54 systematic reviews for full text screening. Currently, two independent review authors are screening these articles and extracting data. Relevant factors for data extraction are e.g. information source, the number and size of studies, details of the included review (location, search time frame) and participants, intervention, and outcome. First evaluations tend to show models regard to transition of care, disease management, care-/case management, telehealth, home-based cardiac rehabilitation, home based care and collaborative intervention. ACCESS is use for data extraction.

Discussion: It is expected that the synthesis will be primarily narrative in nature; however, the exact form will be an iterative process. The aim is to use the data to gain a sense of the research available into the topic area. Finally, we will construct a narrative synthesis that will focus on the interventions and the methodological quality of the various systematic reviews.

Implications for practice: The findings will publish and use for the development of a complex intervention in a feasibility study in the project KARdiologische LandAssistenz.
