Different approaches to improve the aging face were published in the past. Based on the well known techniques of Skoog and colleagues, the innovators of facelift surgery, different procedures were developed in the last years. Short-cut facelifts, minimal invasive and endoscopic facelifts, the MACS-lift and others, were addressed to improve results and produce more natural looking faces.
Despite the advancements of these techniques, there is one concern raised by most patients looking for a face lift surgery. Will I have a natural looking result?
The following description of 76 cases of different facelift patients (age 56.2 +/-12.3y) present a new approach of the three dimensional treatment strategies including dermis-fat-grafts, autologous fat tissue filling, different SMAS-plications but also hyaluronic acid injections in order to augment various structures in the face and counteract volume deficits which could produce a sad and more two-dimensional result. The patients received a volume augmentation mainly in the malar area (n=75), the nasolabial triangle (n=48), oral-mental grove (n=17) or elsewhere as discussed preoperatively with them (n=3).
The patients received a questionnaire in order to collect subjective results according to patient satisfaction. Furthermore, the course was compared with their pre-operative pictures one year after surgery was performed.
Results demonstrate an overall patient satisfaction of 97.3 % (+/-3.7). Comparing the pictures of patients who underwent the three-dimensional facelift procedure, more natural results were found in the follow-up time. However, in order to quantify volume augmentation, 3D-Videoscans are necessary and therefore constitute the next step of our studies being performed at the present time.
