In the last decade fat tissue transplant has been approved as a safe and reliable method for various indications in the field of breast surgery [1]. The Berlin Autologous Lipotransfer (BEAULI) uses a water jet-assisted liposuction can be regarded as one of the youngest evolving fat grafting methods after its first report in 2010 [2]. Several specialized European centers in Germany, Finland, Italy and France started to network and established specific therapeutic protocols for either aesthetic or reconstructive patient groups [3]. Up to date over 560 patients have been successfully treated with over 1400 large volume (>200 ml) lipotransfers for congenital breast asymmetry or hypotrophy, tuberous deformities or soft tissue defects after oncologic breast surgery. Furthermore a specific protocol for patients with severe capsular fibrosis has been developed based on long-term clinical and radiological evaluations [4], [5]. The latter results emphasize, that a „de novo“ breast reconstruction after primary ablative surgery is possible within 3–5 procedures of the BEAULI™ Method [6]. These are combinable simultaneously with other oncoplastic techniques (e.g. contralateral mastoplasty, reconstruction of the NAC). This reconstructive treatment is gradually attained after 12–18 months at least. In 4 years of experience an increasing demand is actually noticed. Although further studies are certainly required to determine definitive international guidelines for lipotransfer in aesthetic and reconstructive breast surgery, it might be regarded as a complementary technique or even promising alternative to conservative methods.
