Objectives: Complex forearm trauma often associates open cominuted fractures with extensive neuro-muscular damage. Even with primary reconstructive treatment the outcomes can be disheartening due to the long recovering period and secondary surgeries.

Method: The author presents the case of a driver with household inflected trauma of the dominant forearm with extensive damage to both median and ulnar nerves and late reconstructive surgery.

Results: The functional outcome was good despite the tardiness of the nerve graft permitting the return in the work field at 6 months following last surgery.

Conclusion: Even for late presentation cases the reconstruction of the forearm nerves should be attempted as this is the paramount condition for functional recovery.
