Efficacy of a topical treatment of skin diseases is influenced by the individual function of the epidermis and the outer layers of the dermis. Especially the stratum corneum, the keratinous unenlivenend outer layer of the epidermis, is forming the barrier against external toxic and efficacious substances. The architecture within the epidermis, the anatomical and functional interdigitation with the corium, the coherence of organized cells and matrix structures show typical alterations during the development of skin disease and are subject to continuous changes during emergence and healing.

Using the examples of eczema, psoriasis and bullous dermatoses at various stages of the disease, the patho-anatomical and functional changes of the skin are demonstrated, and respective specific requirements for an optimal treatment with topical drugs are derived. Innovative drugs with the potential for topical efficacy will be addressed.
