Objective: To investigate the clinical value of surgical treatment in comprehensive treatment of early stage nasopharyngeal carcinoma.

Methods: Based on the case selection criteria,patients with early nasopharyngeal carcinoma were divided into surgery group and the conventional group according to patients’ wishes. Surgery group were treated with surgery plus radiochemotherapy while conventional group were treated with radiochemotherapy.

Outcome indices:

1.
5-year survival rate and disease-free survival rate
2.
Total tumor dose of nasopharynx
3.
Incidence of xerostomia.

Results: By Oct. 2006, total of 95 cases were enrolled in the study, including surgical 32 cases and the control 63 cases.

The cut-off date were Nov. 2011. The median time of follow-up was 67 months. The follow-up rate was 98.95% for 1 case was lost. 13 cases died during the follow-up.There was 1 case died in surgery group and 12 cases in the control group.

95 cases had a 5-year overall survival rate 88.42% and a 5-year disease-free survival of 85.26%. In surgical group, the 5-year survival rate and 5-year disease-free survival rate was 100% and 100%, respectively. In control group, the 5-year survival rate and 5-year disease-free survival rate was 82.54% and 77.78%, respectively.

The dose to nasopharynx in surgery group and control group were 63.91 ± 5.47Gy and 71.11 ± 3.44Gy, respectively. The dose of surgical group were significantly less than that of the control group.

The incidence of xerostomia in surgical group was significantly lower than that of the control group(84.21% vs.25.00%,P=0.000).

Conclusion: The surgery combined with concurrent chemoradiotherapy is a effective comprehensive therapeutic interchange program for early stage nasopharyngeal carcinoma.
