Stapes surgery with implantation of three different types of stapes prosthesis, a retrospective study

Objective: The objective of this study was to present our experience concerning the different surgical techniques and the different types of stapes prostheses we used in stapes surgery.
Method: We analyzed retrospectively the postoperative hearing results obtained in stapes operations with implantation of three different types of stapedial pistons. We studied a total of 154 stapes operations, 143 stapedotomies and 11 stapedectomies. In 49 cases a Causse-type Teflon piston with a diameter of 0,6 mm was inserted, in 45 cases we used a Fisch-type Teflon and stainless steel piston with a diameter of 0,4 mm and lately in 60 cases a Fisch-type titanium piston with a diameter of 0,4 mm was inserted.
Results: Pure-tone audiometry with appropriate masking was performed approximately 6 weeks postoperatively. The average postoperative ABG was calculated as the postoperative air PTA minus the postoperative bone PTA. Although the Fisch Teflon and stainless steel 0,4 mm piston gives a somewhat poorer result in the 0-10 dB range, the difference is not significant, and the results in the 0-20 dB range get comparable equal.
Conclusion: The material and the shape of the prosthesis does not influence the results of the operation in a key way. The experience and the manual expertise of the surgeon remain very important factors in providing a successful stapes operation. The choice between the different types of stapes prostheses depends a lot on the usage and preferences of the surgeons.
