Introduction: Medical care is based on three columns: the general practitioners are in the first line for primary diagnosis and therapy. They transfer patients to the specialists in private practice in more complicated cases. This sequence is valid even after regular consultation hours and is organized by the KV-emergency service.

The medical specialist can send problematic patients for further check up and treatment to the outpatient wards of hospitals, which deal with emergency cases as well. Aim of this investigation was the evaluation of outward patients at the ENT-Department during one year.

Material and methods: The prospective study at the Department of Otorhinolaryngology, Head and Neck Surgery Bad Hersfeld started in April of the year of 2013 and ended in March 2014. All outpatients were registered and analyzed according several items such as personal data, the reason, time and urgency for presentation.

Results: All in all 11,002 patients were enrolled in this study. There were 6,463 elective cases in the regular office hours, however 936 patient came as emergencies during this period. After the consultation hours 703 emergencies were treated at our department. In the weekend 799 patients asked for medical care.

Evaluation of the acuteness of the emergencies demonstrated that 79% of the patients did not require immediate treatment with diagnosis e. g. rhinitis, ear wax etc. Only 1% of the cases afforded instantaneous management such as acute dyspnoea, posttonsillectomy hemorrhage. The average time afforded for patient care was 35 minutes; patient had to wait for range of 0–30 minutes in most cases.

Conclusions: In the rural district Hersfeld-Rotenburg with nearly 120,000 inhabitants our department plays an important role for the management of patients with ENT-problems. During the investigation period nearly 10% of all people in this district made demand on this service mathematically. Especially the emergency treatment was frequently used, however the medical reasons did not justify the presentation in the majority of cases.
