Introduction: Slum-dwellers in Dhaka, Bangladesh are imperiled to a lot of risk factors for gastric pain concerning their nutrition, lavatories, source of water supply and the number of people living in one area. The gastric pain can be due to several disease patterns, such as gastritis, gastric ulcer, viral or bacterial infections. The objectives of this analysis were to examine associations between living conditions as well as environmental factors and gastric pain in a slum population in Dhaka.

Method: Data were used from a prospective cohort study investigating issues of health and their determinants in slum areas in Dhaka. They were collected by an oral standardized questionnaire from 1.938 slums dwellers. Two approaches were finally implemented: analysis of individual data using multivariable logistic regression and analysis of household information using a longitudinal survival analysis.

Results: For this survey 1.938 persons of 10 to 90 years of age were interviewed. The individual data analysis showed that 8.8% suffered from gastric pain. In most of the households only one or two persons had gastric pain. The prevalence of gastric pain depended on the time of questioning. Using multivariable logistic regression significant differences were detected: persons with a diet containing a high proportion of meat had a 1.79-times (95% CI: 1,10-2,93) higher chance to get gastric pain. The chance to suffer from gastric pain rose with higher age; people who were 50 years and older had the highest risk (OR: 3,0; 95% CI: 1,35-6,53). Results of Cox-regression showed that the chance to get gastric pain was associated with living conditions, such as number of people sharing one drinking-water source and number of people living in one room. People sharing their drinking-water source with more than 100 people had a 1,49-times (95% CI: 1,20-1,86) higher chance to suffer from gastric pain compared to people who shared their water with less than 50 people.

Discussion: Due to fact that only one or two persons living in one household suffer(s) from gastric pain, it can be assumed that contagious diseases are not the major cause for gastric pain prevalence in the polled slum population. Information bias may have influenced the results because only one person was interviewed for all household members. The results show, that environmental factors and living conditions may have an impact on gastric pain.
