Development of clinical practice guidelines in paediatrics in the Czech Republic

Background: There is a long tradition in development of clinical practice guidelines in paediatrics (PCPGs). A little is known about the methods of development, adaptation, implementation and evaluation of PCPGs and their impact on the paediatric health care.
Objectives: We have analysed process of development, adaptation, implementation, and evaluation of CPGs in paediatrics in the Czech Republic.
Methods: We searched relevant sources. Eligible CPGs were those focused on healthcare in childhood. A questionnaire survey to obtain more specific information was conducted. All analysed PCPGs were assigned to one of three categories:
- 1.
- PCPGs developed by paediatric and neonatal societies;
- 2.
- PCPGs developed by other societies;
- 3.
- CPGs that include also recommendations for paediatrics.
Methods of development, adaptation, dissemination, implementation and evaluation were analysed as well as ethical aspect, costs and management of conflict of interest.
Results: There are 112 medical societies in the Czech Republic in total. 23 societies have been developing PCPGs. Methodological terminology used is inconsistent and confusing. Specific information on methodology is very scarce and very often not explicit. There is no systematic approach to evaluation of quality of PCPGs, up-dating and declare conflicts of interest. Clinical audits focused on health care quality improvement measures are also missing.
Discussion: Systematic approach to development, adaptation, implementation and evaluation of PCPGs is missing and national strategy is needed.
Implications for guideline developers/users: A set of comprehensive recommendations on methodology and different aspects of PSPGs for guidelines development groups and other stakeholders, based on the study, will be made.
