Background: In 2008, the German Guideline program in Oncology (GGPO) was launched under the auspices of the German Cancer Society, the German Cancer Aid and the Association of the Scientific Medical Societies. Since a major goal is to provide an independent funding for guidelines development estimating the costs for each planed guideline in advance is crucial for the GGPO.

Context: An important part of the application process consists in estimating costs for the development process. Frequently, this is a challenge for Guideline Development Groups(GDGs) especially if there are no experiences from a former process. Therefore, GGPO provides a matrix for fixing resources for certain development steps as support. Nevertheless, there is variation between the guideline project plans basing on different work models, methodological expertise of involved people, the size of the GDG and the numbers of key questions.

Description of best practice: The following aspects will be described and explained by examples: Introducing the GGPO matrix for estimating costs, Comparing calculations of 15 guideline projects, Presenting average costs for defined guideline development steps, Giving examples for deviations from ex ante calculation in ongoing projects, Discussing different resource models and possible savings.

Lessons for guideline developers, adaptors, implementers, and/or users: Based on the experiences of the GGPO during the last four years we will point out which points should be considered for a resource plan being as realistic as possible. Furthermore, a discussion about different resource models and possible savings will be stimulated.
