Background: The project „Arzneimitteltransparenz im Pflegeheim“ was implemented in connection with the project „Arzneimittelinitiative Sachsen-Thueringen“ (ARMIN) initiated by AOK PLUS between 2020 to 2022. “ARMIN” made important contributions to digitalization and interprofessional collaboration between general practitioner and pharmacies in the pharmaceutical care module based on a server-supported, digital medication plan exchange. As part of the model project ARMIN, insured persons with long-term care in inpatient care facilities were not eligible to participate. Doctors have repeatedly expressed their desire to open the “ARMIN Project” to these insured people.

Materials and methods: The “Arzneimitteltransparenz im Pflegeheim” project took into account the further networking of service providers by providing an electronic medication plan for a vulnerable target group. The existing technical infrastructure – implemented in ARMIN – was expanded to include the connection of two inpatient nursing homes. The aim was to provide and exchange an electronic server-based medication plan between the caring service providers in a selected pilot region. This is intended to provide everyone involved in the care process with transparency about the overall medication of the insured persons enrolled in the project.

Results: The project was able to collect practical evidence for the digital medication that will be required by law from 2025. It has made it clear that the existing regulations on the Bundeseinheitlichen Medikationsplan (BMP) respectively for the elektronischen Medikationsplan (eMP) ) cannot reflect the care situation of insured persons receiving inpatient care in nursing homes. In particular, connecting nursing homes to the telematics infrastructure is of far-reaching importance. Only in the course of this can the basis for secure communication between individual service providers be created and implemented.
