Background: The project ARMIN (“Arzneimittelinitiative Sachsen-Thüringen”) was implemented from 2014 until to 2022. It was a joint model project in accordance with the law of the statutory health insurance (SGB V), established by the AOK PLUS, the statutory health association and pharmacist association in Saxony and Thuringia. The content of ARMIN was a pharmaceutical care concept to be implemented jointly by general practitioners and pharmacies. Three aspects were core elements of this project:

- Modul 1: non-proprietary name prescribing of medications,
- 2.
- Modul 2: and the rational use of medicines by using prescription guideline,
- 3.
- Modul 3: a structured medication management.
In ARMIN, a reduction in drug risks, an increase in adherence to therapy and an improvement in the quality of care for certain groups of insured persons should be achieved while at the same time reducing costs.
Materials and methods: Datasets were developed and integrated into the local software of physicians and pharmacies to establish non-proprietary name prescribing of medications and to improve the rational use of medicines by using prescription guideline. For medication management, processes and responsibilities were developed as well as a technical infrastructure to enable an electronic exchange of patients’ medication data.
Results: Around 550 general practitioners and 900 pharmacies took part in this project. Around 330 doctors and 340 pharmacies implemented digitally supported medication management. They have cumulatively enrolled and cared for more than 9,000 patients.
Conclusion: In the contribution to the conference, the results of the ARMIN project, the external evaluation and the lessons learned from AOK PLUS’s perspective are summarized and presented. In ARMIN, a promising interdisciplinary concept has been developed and implemented. Defining responsibilities and integrating the components into the local software of the physicians and pharmacists facilitated implementation.
