Fibrin glue to secure a scleral tunnel incision after phakic anterior chamber intraocular lens implantation

Objective
A leaky tunnel incision is a rare complication after anterior chamber phakic intraocular lens implantation (pIOL). The early application of fibrin glue may help if a leckage of the incision in combination with low intraocular pressure, reduced visual acuity and a higher risk of infection may occur.
Methods
Three patients with high myopia underwent implantaion of a phakic anterior chamber intraocular lens (Verisyse) with a sutured scleral tunnel incision. Postoperativ treatment consisted of steroid and antibiotic eyedrops. After an uneventful early postoperative period, they presented with reduced visual acuity, significant loss of intraocular pressure and choroidal detachement. The anterior chamber depth was normal and the pIOL was not dislocated. Even after a reduced steroid therapy there was no adaptation of the tunnel incision.
Results
After the treatment with fibrin glue (Tissucol Duo® Baxter) the intraocular pressure increased to normal conditions and a fast improvement of the visual acuity was recorded.
Conclusions
In case of leaky tunnel incisions after phakic anterior chamber lens implantation the use of fibrin glue might be a efficient and successful treatment to prevent further complications.
