Objective

In eyes with Fuchs heterochromic cyclitis (FHC) we find an intraocular formation of antibodies against the causative rubella virus. In eyes with an intermediate uveitis in multiple sclerosis (MS) an intraocular antibody formation against measles-, varicella zoster-, herpes simplex- and rubella virus is also seen. We investigate the difference between the immune reaction against the causative antigen and the polyspecific accompanying intraocular immune reaction.

Methods

The aqueous humor of 54 eyes with a clinically defined FHC, 15 eyes with intermediate uveitis in MS and 50 eyes with a cataract as control was investigated. The albumin and IgG concentration was analysed in aqueous humor and serum, as well as the antibodies against measles-, rubella-, herpes simplex- and varizella zoster-virus. Antibody formation was calculated with the Antibody-Index (AI), the corrected Goldmann-Witmer-Index. The quantity of the intraocular antibody synthesis was characterized as the specific fraction, Fs.

Results

The rubella-AI of the 54 eyes with FHC was between 1.5 and 309 (median: 20.6). The rubella-AI of the 15 eyes with intermediate uveitis and MS was between 0.7 and 35 (median: 3.0) .The cataract control group had an AI between: 0.6 - 1.4 (median: 0.94) Frequently, at the same time an increase in the measles-, varicella zoster- and herpes simplex-AI was observed in eyes with an intermediate uveitis with MS but not in FHC. The calculation for the specific rubella fraction in aqueous humor was for eyes with FHC about 40-fold higher than in eyes with intermediate uveitis in MS.

Conclusions

The isolated intraocular rubella antibody synthesis in eyes with FHC indicates an immune response of the eye against the causative, persisting virus (confirmed by PCR). In contrast the frequent measles-, rubella- and varicella zoster/herpes simplex antibody synthesis in eyes with an intermediate uveitis with MS at low concentration is a polyspecific immune response due to the network properties of the immune system.
