In the Baltimore Eye and Egna-Neumarkt Study an association was found between diagnosis primary open-angle glaucoma and systemic hypertension. In disturbing the autoregulation of the optic nerve head and presenting of atherosclerotic changes of the retinal vessels, systemic hypertension plays an important role according to the vascular or ischemic hypothesis of glaucomatous pathophysiology. In addition, an insufficient perfusion pressure during antihypertensive therapy could accelerate a glaucomatous progression. Therefore, systemic hypertension will be discussed in influencing glaucoma therapy and glaucoma diagnostic.
