Operative treatment of optic pit with central serous retinal detachment. Results of P. Stradinš Clinical University Hospital experience. Clinical case

Purpose: Share our clinic experience of operative treatment of optic pit with central serous retinal detachment and understand the effectiveness of the method with 23G pars plana vitrectomy without ILM peeling and following C2F6 gas tamponade for treatment.
Methods: Analyze the case of the optic pit with central serous retinal detachment. Evaluate symptoms and their intensity after surgery, with a LogMAR chart to get to know visual acuity, use a Heidelberg Spectralis OCT device to get data of central retinal thickness (CRT), retinal thickness superotemporally at 45` within 2,2 mm from optic nerve head (ONH) center (there was localized narrow detachment channel, which connected optic pit and central detachment) and compare obtained data before and after surgery.
Results: Initially, BCVA on a LogMAR chart was 0.5 in the left eye (with +3.0 D sphere correction), 3 and 5 months after surgery it became 0.9/1.0. CRT before surgery was 1,222 µm, retinal thickness superotemporally at 45` within 2,2 mm from ONH center- 737 µm and 3 months after operation CRT decreased to 422 µm, but 5 months past to 420 µm and retinal thickness superotemporally at 45` within 2,2 mm from ONH center reduced to 470 µm.
Patient complaints decreased, and visual acuity improved. The condition of the central retinae improved (CRT decreased), but the serous detachment remained. Retinal thickness superotemporally at 45` within 2,2 mm from ONH center decreased, but detachment, which performed the functions of a channel, was not fully closed.
Conclusions: The experience of our particular case is positive, but not ideal. The surgical method with 23G pars plana vitrectomy without ILM peeling and following C2F6 gas tamponade in our case is effective, it improved the patient's visual acuity, and retinal parameters and reduced complaints.
Considering the rare occurrence of the disease, additional studies are needed to understand better the effectiveness of various surgical treatment approaches in the treatment of the disease.
