Purpose: Application of monovision with two RayOne enhanced monofocal intraocular lenses EMV (toric and non-toric) from Rayner (West Sussex, UK) to increase the depth of focus in a high myopic patient.

Report of case: A 56-year-old patient with high bilateral myopia (AL OD 28.14 mm; AL OS 27.26 mm; subjective refraction OD -13.5 -2.0 x 95°, OS -9.00 -2.00 x 160°) and moderate corneal astigmatism on dominant eye (DE) after cataract surgery had implantation in his DE enhanced monofocal lens RayOne EMV toric RA021T from Rayner with emmetropization and in his non-dominant eye enhaced monofocal lens RayOne EMV model number RA0200E with target refraction - 1,50 D. This combination aims to correct one eye for distance vision and the other eye for near, resulting in a full monovision approach with increased range of focus, thus allowing a broader spectrum of visual fields.

Results: Post-operative subjektive refraction 1 week after surgery shown manifested target refraction allowing in binocular vision logMar +0.1 (Decimal 0.8) and providing good vision at intermediate and near distances. Intraocular axial position of the toric lens in DE 1 week postoperatively was stable anticipated its supposed rotational stability. Subjective refraction, uncorrected and corrected visual acuity (VA), binocular [logMar] for distance, binocular for intermediate and near vision, as well as defocus curve 1, 3 and 6 months after surgery are still in evaluation.

Conclusion: Even in a high myopic patients application of full (1.5 D) monovision with two RayOne enhanced monofocal lenses EMV from Rayner proves to be a good choice, aimed to deliver a non-diffractive, high quality vision with an increased depth of focus.
