Purpose: To compare the efficacy of customized corneal crosslinking (CXL) with standard CXL.

Methods: Eyes with documented progressive keratoconus were treated with customized CXL or standard CXL. Customized irradiation patterns were individualized to the patient’s posterior shape of the cornea with radiant exposure levels ranging from 5.4J/cm2 up to 10J/cm2. The control group received homogenous irradiation with a total energy of 5.4J/cm2. Statistical analysis included tomographical parameters, BSCVA, anterior segment OCT and endothelial cell count.

Results: Tomographical regularization was stronger after customized CXL than after standard CXL (KMax reduction -2.0 ± 1.4D vs. -1.1 ± 1.4D). Significant flattening (>1D) was observed in 70% of the eyes in the customized CXL group but only in 40% in the standard group. A flattening of >3D occurred 4 times more often after customized CXL.

Conclusions: Customized CXL shows better regularization and flattening than standard CXL.
