Increased rate of cam deformity in early stage nontraumatic osteonecrosis of the femoral head

Objectives: To investigate the correlation between the early stage idiopathic osteonecrosis of the femoral head (ONFH) and the cam femoroacetabular impingement (FAI).
Methods: One hundred and thirty-four hips from one hundred and twenty-two idiopathic ONFH patients were matched by age and gender with one hundred and thirty-four hips from one hundred and thirty healthy subjects. The alpha angle the hips were measured independently in both anteroposterior and frog-leg lateral radiographs. Stage- and gender-stratified analysis of the average alpha angle and the prevalence of the cam FAI (alpha angle >50°) was performed.
Results and conclusion: Compared with matched controls, a significantly more cam deformity was identified in the anteroposterior radiograph of Ficat stage III ONFH patients and in the lateral radiograph of Ficat stage II and III patients (P<0.001) (Figure 1). Of note, the Ficat stage III patients obtained a remarkable increase of the average alpha angle compared with the Ficat stage I and II ONFH patients (P<0.001). Such increase of the average alpha angle was independent of gender of patients when compared with matched controls (P<0.001).
In conclusions, these data indicate an increased rate of cam FAI occurring in early stage nontraumatic osteonecrosis of the femoral head.
