Introduction: A lot of patients have suffered from lesions in the medial aspect of the the thumb and complained of functional disability and cosmetic disfigurement, beside their discomfort toward other methods of reconstruction. I introduce the first web space flap as a solution for all these problems.

Aim: To evaluate the efficacy of first web space flap in repairing the lesions of the medial aspect of the thumb and its advantages in comparison to other options.

Material and Methods: A prospective study applied on patients of Menofia university hospital with trauma to the medial aspect of the thumb. They were divided into two groups: Group A (repair with first web space flaps) and group B (repair by other variant distal flaps). A close observation of the function and cosmetic appearance of the repaired thumbs was fulfilled. And a questionnaire concerning the satisfaction and the comfort of the patient toward each repair method was fulfilled also.

Results: 34 patients were included in this study, 13 patients were in group A and 21 patients were in group B. The patients of group A showed better progress in regaining function and cosmetic appearance than group B. Also the patients in group A expressed more satisfaction and comfort than group B patients. Conclusion: First web space flap is a reliable and good option for repair of lesions in the medial aspect of the thumb.
